Getting started

This application should be completed by a serving or former member of the Australian Defence Force, an eligible civilian, or a person on a special mission who would like to know if they have rendered qualifying service. Important information

The information you have provided on this application is required to determine whether you have rendered Qualifying Service under the Veterans' Entitlements Act 1986. Note: you can have Qualifying Service under the VEA even if your service is covered for compensation purposes under the Military Rehabilitation & Compensation Act 2004.

This application is NOT a claim for service pension. Complete Form D0503 (Claim for Service Pension by a Veteran or Mariner) if you wish to claim a service pension.

The personal information provided in this form will be used to update Departmental records. Some of the information may be disclosed to Centrelink and the Australian Taxation Office for the purposes of data matching, the Department of Health & Ageing for income testing in relation to aged care services, and to the Health Insurance Commission for treatment account payments.

During the process you will be asked to attach documents to help the Department process your claim. These may include:

  • Campaign star / medal certificate of award
  • Statement of service
  • Discharge certificate
